Note: If SPJA cannot confirm you have a credential registered under your name, your submission will be denied. * -- Select One -- Yes No I am the buyer Guests Autographs - Minor Participant Waiver * I understand that, if I am representing a minor (age 13 or under), I MUST:1) be listed on the order with their Child credential and my attendance is required at the autograph session with them;2) have a valid anime expo chibi credential; and 3) have a government-issued photo ID to present at the door during check-in. If I am representing a minor (age 14-17), the minor's name must be assigned as a ticket holder for their credential. If the minor does NOT have a valid government-issued photo ID to present at the door, then I MUST:a) accompany them during the autograph session;b) be listed on the order with their Weekend credential; andc) have a government-issued photo ID to present at the door during check-in. Guests Autographs - Registration Waiver * I understand that if I am selected as a winner for any Guest Autograph Session, I must register using the name provided on the application. If SPJA discovers I have registered under a different name or have assigned the ticket to another person, then my reservation will immediately be null and void. I have read and agree to the above waiver. Guests Autographs - Application Agreement * I understand that I am only allowed to submit one (1) application form under my name. If SPJA discovers I have submitted multiple applications, then SPJA has the right to void all applications and deny my participation. I have read and agree to the above waiver.
